How Do I Qualify
Claiming your Ballarat MyChoice for Living pack - is quick and hassle-free.
Claiming your Ballarat MyChoice for Living package is simple, quick and easy. All you have to do is visit the Council Customer Service Desk located in The Phoenix building, 25 Armstrong Street South (opposite Central Square) with proof of long term Ballarat residency. To qualify, you must be moving into a property that contributes Council rates to the Ballarat City Council. The MyChoice package must be claimed within three (3) months of moving to Ballarat.
Eligibility
Eligibility for the MyChoice package will be recognised to persons who produce one (1) of the following documents:
A signed Contract of Sale: or
A rental agreement of at least six(6) months: or
A letter of employment indicating permanent employment or an employment contract of more than six(6) months duration from your new employer.
AND one (1) of the following documents:
Drivers licence; or
Car registration papers
Students
Students must also produce, in addition to the requirements stated above, documentation from their tertiary institution indicating a period of study of at least one (1) year. Acceptable documents include enrolment forms, fee receipts or HECS details.
How long will it take?
The whole process will take 5 – 10 minutes and you will walk out with a MyChoice pack worth over $22,000! If you cannot get down to The Phoenix building, then fax your details and the required documents to (03) 5333 4117 and the pack will be sent to you by post.
